How do I make an appointment?

Scheduling ahead of time guarantees you will be seen. An appointment gives you priority. You can either call us directly at (323) 953-7330 to schedule or log in to MyChart.

Need to see a doctor today?
While we try to accommodate walk-ins, we aren’t always able to. Same-day appointments are sometimes available. For the best chance of getting seen the same day, call before 9:00 am.

What do I need to bring?

When you make an appointment, you’ll get an arrival time that gives you a chance to complete registration and benefit program screening paperwork.

Otherwise, please have the following ready for every visit:

  • Photo ID
  • Insurance card, if you have coverage
  • A list of current medications
  • Payment method

If you don’t have insurance, enrollment advocates will screen you to see if you qualify for a health plan or discount program. Be sure to bring any application paperwork you receive with you to your first visit.

Gracelight is a contracted partner with Health Care LA (HCLA), IPA. We care for patients enrolled with government-sponsored programs like Medi-Cal, Medicare Advantage, and Medi-Medi through this partnership. Learn more about HCLA.

What if I don’t have insurance?

If you don’t have health insurance, Gracelight offers several options to help you obtain coverage and to help low-income individuals and families pay for their healthcare costs.

Benefits screening: All new patients receive a benefits screening at the first appointment to make sure they are getting the maximum coverage available.

Insurance overview: There are many insurance programs available that can help cover your healthcare costs. Multi-lingual enrollment advocates are available to help you understand what you may qualify for and how those programs work.

Enrollment assistance: Our certified enrollment counselors help complete program applications, make sure patients understand what the programs cover, and facilitate post-application follow-up with Medi-Cal social workers and choosing and HMO.

Sliding Scale Policy: For any uncovered fees, Gracelight offers a discount policy to pay on a sliding scale. Eligibility is based simply on family size and income.

Changing Providers

We want you to feel completely comfortable with us. If you would like to request a different provider, simply give us a call at (323) 953-7330. The Gracelight Patient Access Team will get back to you within 48 hours to honor your request.

Medical Records

Gracelight maintains records of treatment and care for all patients. You have the right to receive a copy of your records at any time.

The easiest way to request your records is through MyChart. Otherwise, please follow these guidelines to place a request by mail or in person.

Once a request is received, it takes about 15 business days to process. Please note there may be a fee for records, depending on how you want to receive them.
