Dr. Nooriya Khokha, Director of Quality
Dr. Nooriya Khokha brings more than a decade of experience in healthcare transformation, quality improvement, and patient-centered care to her role as Director of Quality for Gracelight Community Health. Her work focuses on advancing health equity, strengthening provider partnerships, and improving outcomes for diverse communities.
Before joining Gracelight, Dr. Khokha served as a Clinical Practice Transformation Specialist with the Community Health Network of Connecticut, where she supported practices in achieving Patient-Centered Medical Home recognition and implemented data-informed quality initiatives. She is a licensed physician in India and an NCQA Patient-Centered Medical Home Content Expert. Dr. Khokha earned her M.B.B.S. from Mahatma Gandhi Mission Medical College and her Master of Science in Counseling with a specialization in Gerontology from San Francisco State University, graduating summa cum laude.
Outside of her professional role, Dr. Khokha volunteers with the Jewish Family Service LA Food Bank, reflecting her ongoing commitment to service beyond the clinic. At Gracelight Community Health, she is dedicated to strengthening quality systems, advancing equitable care, and supporting healthier outcomes for the communities we serve.
